Adecco is currently seeking a Case Manager to join our client’s team in the Geriatric Healthcare industry in Toronto, ON. This is a Temporary position offering a competitive wage of $35 per hour. The role requires a dedicated and compassionate individual who has experience working with senior-aged clients, and an understanding of aging-specific complexities such as physical health, cognitive impairment, and mental health challenges.

As a Case Manager, you will initiate services for senior-aged clients, ensuring proper assessment and service response, while collaborating with various healthcare professionals to ensure high-quality care.   • Pay rate: $35/hour.
  • Location: Toronto, ON
  • Shifts: Monday-Friday 9:00am-5:00pm

Responsibilities:

  • Initiating services for senior-aged clients at the point of intake to ensure proper assessment and timely response to their needs.
  • Working with older adults, understanding aging-specific complexities like physical health, cognitive impairments, and mental health challenges.
  • Developing clinically sound case treatment plans, utilizing appropriate assessment tools that are tailored to the needs of the client and program objectives.
  • Collaborating with various organizations and service systems to ensure that services are well-coordinated and meet high standards of quality care and supervision.
  • Identifying problems and potential solutions for resource use and collaborating for service improvement.

Requirements:

  • Must be legally eligible to work and reside in Canada.
  • Completion of relevant post-secondary education (e.g., Geriatric Healthcare, Social Work, Social Services).
  • 3-5 years of experience in a geriatric health setting.
  • In-depth experience and knowledge of geriatric issues, mental illness, and the mental health system.
  • Strong case management and referral skills to effectively link clients to appropriate services.
  • Excellent verbal and written communication skills.
  • Ability to work independently and effectively as part of a multi-disciplinary team.
  • Proficiency in Microsoft Office, Client Information Systems, and web browsing.
  • Fluency in a second language is preferred.
  • A valid driver's license and access to a personal vehicle.
  • Willingness to undergo a criminal background check.
  • Flexibility to work weekends, evenings, and statutory holidays as required.
  • Willingness to participate in an on-call rotation, if needed.
