Currently we have an exciting opportunity for a Registered Nurse to work in our Home Hemodialysis Program: Long-Term Care Satellites, Nephrology Program. The Registered Nurse is responsible for addressing the needs of a unique patient population.

Using a Case Managed approach, the Registered Nurse coordinates clinical activities, acts as a clinical resource/role model for other staff members, and works directly with patients and members of the team to ensure timely and appropriate treatment and necessary follow up.

The Nurse will autonomously assess and educate patients/family members. This individual will closely liaise with the multidisciplinary team.

The Nurse working in the Home Hemodialysis Long-Term Care Satellites, will take a comprehensive approach to providing care for patients which chronic kidney disease, including assessment of patients being referred, training and ongoing primary case management.

As an integral team member of the Nephrology Program, there is an opportunity to participate within the Program by utilizing a chronic disease prevention and management (CDPM) and self-care philosophy, as the Program continues to achieve the deliverables under the Ontario Renal Network (ORN) strategic plan.

This includes championing initiatives such as Person Centered Decision Making (PCD), Access to Kidney Transplantation (AKT) and other relevant and patient centered priorities.

Reporting Relationship: Manager, Home Dialysis and Transitional Care Unit, Nephrology Program
Hiring Salary Range: $39.07 - $56.00

Hours of work: Monday to Friday days

Responsibilities:

  • Performing all aspects of patient care including care paths, patient education, home and community visits, transplant workup, and monitoring quality indicators to ensure treatment adequacy and patient safety is maintained for all primary patients and as assigned
  • Demonstrating a commitment to ongoing professional development within an evolving field
  • Providing assessment, education, training and decision support for patients/family as it relates to independent dialysis
  • Coordinating/facilitating patient appointments including clinic visits in collaboration with clerical team members
  • Engaging the interdisciplinary team in a collaborative approach to professional practice issues and acts in the role of Liaison between all members who participate in the care of the patient and their family
  • Applying relevant research to clinical practice using critical thinking and decision making; utilizing research/evidence based information to recommend nursing interventions
  • Participating in and supporting CQI initiatives
  • Providing a framework /smooth transition for patients who are involved in transitioning to the long-term care dialysis units
  • Candidates will be required to visit the patients in their homes as part of the training and support
  • Meeting “on call” requirements both weekdays and weekends
  • Required to work shifts as scheduled
  • Participating in Nephrology Program projects as assigned
  • Participating in rotational Home Support Nurse assignment
  • Communicating and collaborating with home dialysis vendor
  • Collecting, documenting, and reporting clinical and non-clinical data into Meditech for the purpose of ORRS submission.

Requirements:

  • Registered and in good standing with the College of Nurses of Ontario
  • BLS Provider certified required
  • CNeph Certification preferred. Required to complete CNeph certification or equivalent within 28 months from the hiring date.
  • Baccalaureate degree preferred
  • Minimum of 2 years recent nephrology experience with at least 18 months in hemodialysis required
  • Previous experience in pre-dialysis, transplantation, or home hemodialysis is an advantage
  • Experience with ORN data collection preferred
  • Proficiency in using Microsoft office (Word, Outlook, Power Point and Excel) and Meditech
  • Demonstrated strong ability to assess a patient’s readiness to learn and apply the principles of adult learning while teaching patients and families
  • Demonstrated leadership skills in workload planning
  • Experience in developing/implementing patient care algorithms and patient education tools
  • Demonstrated excellent patient assessment skills
  • Demonstrates competency in chosen profession; consistently demonstrates knowledge and expertise in professional practice and hospital-based standards
  • Engages in continues learning and reflective practice
  • Demonstrates being patient and consumer focused; makes a commitment to meet or exceed the needs of all internal and external customers
  • Excellent communication and interpersonal skills required
  • Demonstrated excellent organizational and planning skills
  • Ability to be flexible and effectively work in busy environment with changing priorities and deadlines
  • Demonstrated initiative and engagement in quality improvement
  • Demonstrated ability to be self-directed and work independently
  • Demonstrates fiscal responsibility
  • Must be able to complete the physical duties of the position, the “Registered Nurse” role description, and the task and duties listed in the “Home Dialysis” standard of care
  • Demonstrated understanding of and commitment to Humber’s comprehensive safety programs and practices, including staff patient, and environmental safety, and participation in regular in-service education in this area; and
  • Valid driver’s license required and access to a personal vehicles for on call and home visits
  • Excellent attendance and discipline free record required
