The CTT RN plays an integral role in identifying barriers to discharge, and working with the healthcare team to develop, communicate and implement discharge plans. The CTT RN must have excellent knowledge of Acute Care and Continuing Care patient flow as well as Community resources.
The position influences the quality of care delivered to clients in Integrated Home Care through evidence-based practice, and supporting knowledge and understanding of discharge planning to interdisciplinary team members. Work is achieved through communication and collaboration within the teams.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families.You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
Current Basic Cardiac Life Support- Health Care Provider (BCLS-HCP).

A minimum of - Experience on Community Transition Team OR 3-5 years in Integrated Home Care Program OR 3-5 years in Medical, Surgical or Geriatric nursing role Demonstrates the ability to build relationships and collaborate with staff from various backgrounds Demonstrates strong leadership in role focused work, strong organizational skills, effective communication and strong computer skills Awareness of Home Care services available to clients in South zone Awareness of AHS policy and process related to Living options Demonstrates the AHS Mission and Values in daily interactions with our teams and Clients Ability to perform all assigned tasks and direct clinical care including but not limited to: Constant adjustment of position (sit/stand) throughout the shift Physical exertion including bending/walking /sitting or standing for long periods of time Keyboarding/mousing documentations for prolonged periods of time
Baccalaureate degree in nursing preferred Connect Care experience preferred Additional education, advanced skills, and clinical preparation across the Seniors Health spectrum (Home Care, Long Term Care, Access Center, Palliative Care and Specialty Geriatric Areas) strongly preferred
