Case Manager - Toronto
Adecco is currently seeking a dedicated and experienced Case Manager to join our client’s dynamic Psycho-Geriatric Multi-Disciplinary Team in Toronto. This role focuses on providing comprehensive, client-driven services to older adults dealing with complex mental health and geriatric issues.
- Pay rate: $30/hour.
- Location: Toronto, ON
- Shifts: Monday-Friday 9:00am-5:00pm | Must be available to work evenings, weekends, and holidays as needed.

Responsibilities:
- Initiate services for senior clients at the point of intake, ensuring proper assessment and timely response.
- Work closely with older adults, addressing complexities related to physical health, cognitive impairment, and mental health.
- Develop clinically sound case treatment plans tailored to individual client needs.
- Collaborate with service systems and organizations to ensure coordinated, high-quality care.
- Identify problems and propose solutions to improve resource utilization and service delivery.
- Provide support and education to family, friends, and caregivers of clients.
Requirements:
- Must be legally eligible to work and reside in Canada.
- Completion of relevant post-secondary education (e.g., Geriatric Healthcare, Social Work).
- 3-5 years of experience in a geriatric health setting.
- In-depth knowledge of geriatric and mental health issues, and the mental health system.
- Excellent verbal and written communication skills.
- Proven case management and referral skills.
- Self-directed and capable of working both independently and as part of a multi-disciplinary team.
- Proficient in Microsoft Office and Client Information Systems.
- Fluency in a second language is a strong asset.
- Valid Driver’s license and access to a personal vehicle.
- Willingness to undergo a criminal background check and work flexible hours, including weekends and holidays.
