Oncology Pharmacist
Accountabilities:
- Work collaboratively with the Pharmacy and Oncology teams to provide patient-centered care
- Provide drug information to physicians, nurses and other clinicians to enhance patient care
- Clinical review and verification of all medication orders for patients in the ambulatory oncology clinic
- Provide education to patients to promote optimal drug use
- Mentor Pharmacy residents, students and interns
- Supervise the preparation of chemotherapy
- Resolve day-to-day clinical and operational issues
- Oncology Clinic operations support
- Participates in oncology program quality improvement initiatives
- Other duties as assigned
- Currently licensed by the Ontario College of Pharmacists (Part A of the Registry)
- Relevant clinical experience in oncology preferable
- Working knowledge of OPIS preferable
- Working knowledge of Meditech preferable
- Membership to CAPhO an asset
- Knowledge of hazardous sterile compounding, including NAPRA standards
- Excellent oral, interpersonal and written skills
- Excellent computer skills
- Demonstrate initiative and a willingness to work collaboratively in a team environment
- Must be able to meet the physical demands of the position
- Excellent attendance, punctuality and work record
