Position Profile:

Humber River Health. Lighting New Ways In Healthcare.

Since opening our doors in 2015 as North America’s first fully digital hospital, we remain unwavering in our belief that we can change the hospital where we work, the community where we live, and the world of healthcare beyond our borders. Serving a community of 850,000 residents in North West Toronto, Equity Inclusivity and active participation in the North West Toronto Ontario Health Team are key initiatives important to our Team.
At Humber River Health, we use a custom combination of technology and clinical expertise to rebuild elements of care. We make technology work for staff and physicians; giving them more time to spend with patients, to eliminate inefficiencies, and to reduce the chance of errors.

Humber River Health is formally affiliated with both the University of Toronto and Queen’s University and committed to becoming a community academic hospital. Clinical Excellence, Optimizing Care through Technology and Community Connection frame our Research Strategy.

Humber River Health’s Nephrology Program is nationally recognized for pioneering the Nocturnal Home Dialysis Program—the first of its kind. The program includes two inpatient medicine units, a satellite service at the Reactivation Care Centre and several clinics (Multi-Care Kidney Clinic, Nephrology Clinic and Dialysis Clinic) designed to support patients in leading independent and active lifestyles and achieving better clinical outcomes.

The Program Director will work in partnership with the Ontario Renal Network (ORN) to align and enhance chronic kidney disease services across the Toronto region, improving patient care quality and outcomes.

Career Experience and Core Competencies:

Master’s Degree in Nursing or related discipline, registration in good standing with the Regulatory College of area of specialty (if applicable), clinical experience in acute services coupled with at least five years management experience working with a medical dyad partner and experience in business planning, resource management, program planning, development, evaluation and managing patient care services is required for this role.
