Health Services - Seniors Services Development

Status: 1 Contract Full Time Position (6 Month Term)
Salary Range: $89,469 - $111,834 per annum
Work mode: Hybrid* see below for more details about this work mode.
Location: 10 Peel Centre Dr, Brampton, ON - which expectation to travel to Adult Day Services sites including Tall Pines and Davis Centre

Hours of work: 35 hours per week

Who we are: We deliver services and supports which allow residents of Peel to age in place. We provide strategic leadership of our Community Support Services which includes Adult Day Services.

What you will do in this role:

  • Collaborate with the inter-professional team to support and provide services that meet the individual needs of both clients and their caregivers
  • Maintain contact with other health care providers, community support services and/or social services in order to develop and implement effective plan of care and support for clients and caregivers
  • Collaborate with clients and caregivers to support current and future transitions including preparing for future health and social services
  • Implement and organize the delivery of specific services within the community support services e.g. Adult Day Service in collaboration with the Community Support Services (CSS) leadership team
  • Ensure compliance with relevant Regional and Provincial Acts and standards, Accreditation and Ontario Health agreements
  • Participate in meetings and committees internal and external to the organization
  • Advocate on behalf of clients and caregivers with internal and external stakeholders such as Public Guardian and Trustee, Ontario Disability support Program, Ontario March of Dimes, Ontario Health atHome etc.
  • Maintain and complete statistical data and monthly reports
  • Complete documentation as required in the client’s electronic health records in accordance with the College of Social Worker and Social Service Standards
  • Plan, develop and implement support groups, caregiver retreats, and other events based on the needs of clients and/or caregivers
  • Participate in the continuous quality improvement (CQI) program for Community Support Services to identify, analyze and provide recommendations on trends, risk and opportunities
  • Provide ongoing therapeutic counselling/follow-up to support clients and caregivers that will meet their individual needs
  • Supervise students, as required
  • Travel between various Regional sites

What the role requires:

  • A Bachelor of Social Work degree from a recognized university
  • Membership in good standing with the Ontario College of Social Work and Social Service Workers
  • Prior community development/ work experience in community services with seniors
  • Counselling experience in a community setting, or long-term care or social service agency that serves the elderly
  • Knowledgeable of the psychosocial needs of community seniors and the informal caregiver
  • Familiarity of available community services/programs to assist clients and their families, as appropriate, to assist in navigating the health and social service systems
  • Sound understanding of gerontology, Alzheimer’s disease and related dementia’s
  • Knowledge of relevant Regional and Provincial legislation including the Consent to Treatment Act, Substitute Decisions Act, MFIPPA, PHIPA, Consent and Capacity Board, Powers of Attorney and policies and procedures
  • Combination of education and experience may be considered

  • Demonstrated skills in assessment, facilitation and counselling
  • Strong communication skills both written and verbal to be able to effectively work with clients, caregivers, staff, volunteers,internal and external resources
  • Effective conflict resolution skills to assist clients, caregivers, and family members
  • Strong leadership skills
  • Facilitation skills conduct counselling and support groups and do presentations to promote community support services
  • Strong problem-solver to be innovative and creative in developing strategies that successfully meet the needs of clients and caregivers
  • Polished organizational, planning, and time management skills
  • Demonstrate a commitment to diversity, equity, and inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias

Work Mode & Job Location: 10 Peel Centre Dr, Brampton ON is the base location with expected travel, as required to include Tall Pines, 1001 Peter Robertson Blvd, Brampton ON, and Davis Centre, 80 Allan Dr, Bolton, ON, as well as other Regional sites on occasion.
