Our services are delivered to help transform the lives of people affected by mental illness and addictions. Our goal is to tailor treatment to each individual, providing client-centered, comprehensive, coordinated care. As part of the Child, Youth, and Emerging Adult Program (CYEAP), the Child Youth and Family Services (CYFS) provides care and treatment to children and youth between the ages of 6 and 24.
Clients and their families served by CYFS are offered specialized services and customized care across a day treatment/day hospital and outpatient continuum of care. Position Description AMANI: Mental Health and Substance Use Services for Black Youth formerly known as the Substance Abuse Program for African Canadian and Caribbean Youth (SAPACCY) is the first-ever Canadian hospital and community partnership of its kind, focused on advancing care for Black youth.
This model of care provides safe, accessible, Africentric and racial trauma-informed support for Black youth (between 12 and 29 years old) and families/caregivers who are impacted by mental health and/or substance use health concerns, and is currently available at eight (8) locations across Ontario.
The service is seeking a full-time, contract (1 year) Social Worker who will report to the Senior Manager of Child, Youth and Family Services and work collaboratively with other clinical services within the CYEAP and across CAMH, including working collaboratively with the Black Health Strategy team at the Provincial System Support Program at CAMH.
This position will function as a clinician case manager within an interprofessional Consultation-Liaison Service and Virtual Care Team (VCT), supporting Black youth (and families/caregivers as appropriate) between the ages of 12 and 29 years old, who are navigating mental health and/or substance use health concerns.
You will also provide consultation to other professionals working with this population across CAMH, and consult and collaborate with the VCT on a weekly basis to support clients in navigating to appropriate hospital- and community-based care providers and/or resources as needed, including close collaboration with AMANI Community Partner Sites.
The successful candidate will also be responsible for monitoring, triaging and processing referrals, coordinating and facilitating various clinical/educational group sessions on an as-needed basis, and documenting participant progress. You will provide client-centred and family-centred mental health and substance use health care, including ensuring the safety and security of clients and maintain standards of social work practice as per your professional regulatory body.
The incumbent will deliver care within a model that recognizes the lived experience of diverse Black youth and communities who have faced systemic oppression and anti-Black racism as it relates to many aspects of their lives inclusive of the social determinants of health.
Clinical interventions will include culturally responsive assessment and treatment planning, case management, system navigation, family support, along with individual and group counselling. The incumbent will support program development and clinical evaluation and be an active participant in continuous quality improvement, research activities support and supervision of students.
Collaboration with community partners seeking support and shared care for complex clients is a significant part of this role. You will support a healthy workplace that embraces diversity, encourages teamwork and complies with all applicable and regulatory requirements.
Some evening hours are required as is work in the community. Other related duties may be assigned as required. This position is located at 80 Workman Way. The successful candidate will possess a Master’s of Social Work degree combined with a preferred 3 years of post-graduate work experience and be registered and a member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
You will be eligible for an adjunct faculty appointment with Faculties of Social Work that have affiliation agreements with CAMH. The candidate will possess demonstrated skills and experience in working with diverse Black youth and communities, in addition to lived experience as a member of Ontario’s diverse Black communities.
Demonstrated experience as a role model is essential. The successful candidate will possess demonstrated skills and experience in the assessment and treatment of substance use concerns and mental health treatment inclusive of individual and group counseling.
Experience in clinical assessment, individual and group-based counseling, client case management, and basic research skills such as data collection and data entry is essential. Proficiency in motivational interviewing, brief therapy modalities, harm reduction principles, and trauma-informed care is required, along with knowledge of equity, diversity, inclusion and accessibility issues in clinical settings.
Capacity to work with complex high risk clients and demonstrated excellence in applying best practices to culturally responsive clinical care is required. Excellent documentation, organizational and interpersonal skills and the ability to work effectively in an integrated clinical research environment where standardized data collection is used to evaluate outcomes is essential.
The ability to work in a dynamic and changing interprofessional environment that supports best practices, innovation, accountability and team collaboration to improve quality of care is required. Knowledge of community resources for African, Carribean and Black youth in mental health and substance use treatment is essential.
Experience working with Black male youth is an asset. Strong communication skills (verbal and written) combined with computer literacy are required. The ability to work effectively as a team member within and across services in CYEAP and CAMH is necessary.

Bilingualism (French/English) or proficiency in a second language would be an asset. Availability to work some evening hours is necessary.
